Extended Care

Sometimes a helping hand is all people need to maintain independence and continue to live at home. Our Extended Care program can be that helping hand. Extended Care provides basic health care and personal care for those who may need support in maintaining their independence, but do not have skilled medical needs and are not totally homebound.

"Extended Care helped me to keep mother at home instead of in a nursing home. It helped me so much and it gave mother another dimensions. The aide was able to get mother to do things that I* wouldn't normally be able to do. She helped her bathe and be independent. Most of all, she gave mother a lot of gentle, compassionate care."

It is not unusual to have services requested by an out-of-town child who is looking for ways to support a parent who needs assistance, but wants to continue to live at home. As one daughter recently wrote about Extended Care: "Went beyond the job description - friendship was the greatest gift to my dad."

Certified aides and homemakers provide the following support services under supervision of nurses:

  • Personal care assistance
  • Shampoo and hair care
  • Light meal preparation
  • Laundry
  • Light housekeeping
  • Companionship
  • Shopping and errand assistance
  • Transportation to appointments
  • Assistance with exercise and ambulation
  • Nursing Services (RN, LPN) are also available through Extended Care
