Documents and Paperwork
  • OB Pre-Admission Data: Note: Patients typically receive this form during their 30-week visit with their provider. It's also acceptable to print this form and return it to the hospital. Proof of insurance and identification will also be verified upon admission even if the pre-admission form is completed ahead of time.
  • Birth Certificate: The Birth Certificate needs to be filled out and given back to your nurse before you leave the hospital. The birth certificate worksheet will be filled out by you and returned to staff before you are discharged after your infant is born. The hospital does not automatically send your birth certificate to your home after this paperwork is filled out - we simply submit it to the State Vital Statistics office. You will need to obtain a copy of the birth certificate on your own after you are discharged by going to the Lawrence County Courthouse (Deadwood, S.D.) a week or two after your baby is born.
  • Social Security Card: If you want your baby to have a Social Security Card, you must request this by checking the box on the Birth Certificate paperwork before discharge after your baby is born.
  • Baby Pictures: We will put your baby’s photo on the internet for free. If you choose to have this done, you must fill out a consent form.
  • Paternity Affidavit: If you are single and want the father’s name listed on the birth certificate, mother and father must sign an affidavit with a notary present. Both parents will be required to show a photo identification. Information for this process will be explained by staff and a video is available for you to view when you are admitted and/or after your baby is born. Sometimes, if the health information department is not here or a notary is unavailable during your hospital admission, you will need to return to the Medical Records department after you are discharged to complete the necessary paperwork with the Records notary.

Note: To protect all babies, any items you bring for baby care will be kept in your room and will not be taken into the nursery.

We strongly suggest that you limit the number of visitors to allow you and your baby as much time as possible to get to know one another. Your nurse will need this time to teach you and your husband/support person how to care for your little one.

For our new mothers and fathers, quiet time at Spearfish Regional Hospital labor and delivery is between 2 and 4 p.m. daily.

Breastfeeding Information
Breastfeeding classes are available at Spearfish Regional Hospital the second Tuesday of each month. Cost is $10 per couple. For schedule and registration information, call (605) 644-4420.

Rapid City Regional Hospital also offers Lactation Consultant Services.

Common Tests to Expect During Your Pregnancy
To ensure a safe delivery for your baby, your doctor may order various tests or screens. Some of the commonly ordered tests are listed below:
  • Gestational Diabetes Screening: The American Diabetes Association recommends that all pregnant women be screened for gestational diabetes at 24-28 weeks gestation. Gestational diabetes is one of the most frequent complications of pregnancy. This kind of diabetes develops during pregnancy and often disappears after the baby is born. When it is detected, usually in mid pregnancy, it is essential that your blood sugar be brought and kept at normal levels so that your baby will not be affected. High blood sugar can have adverse effects on your baby.
  • Ultrasound: Ultrasound uses sound waves above the range of human hearing to produce in different intensities, the return image of the fetus on a TV-like screen. The mother-to-be lies on a table and the ultrasound scanner is passed over her abdomen, into the uterus. When the waves strike tissues of ing echoes vary in intensity. The echoes arrange themselves in patterns of light on the screen, thus providing information which can help the doctor evaluate the baby’s size and growth, gestational age, determine baby’s position, and rule out/confirm multiple births. Ultrasound is painless, accurate, and fast. It does not carry the hazard of x-ray exposure, and there is currently no proven risk from modern ultrasound equipment.
  • Non-Stress Test: The non-stress test is done to evaluate the condition of the baby and placenta. It is most often indicated when a baby is overdue or at some risk. The test is performed by placing a monitor over the mother’s abdomen and recording the baby’s heart beat.  When the mother feels the baby move or kick, she indicates so, and later the baby’s movement can be related to the heartbeat.
