Home Health

Nurses, therapists, aides and other specialists provide care in your home to promote independent living and rehabilitation. The Home Health team closely coordinates with physicians to follow treatment plans.

Regional Health Home+ Home Medical Equipment

Home+ Home Medical Equipment offers a full range of medical equipment and supplies to make home care easier for patients and their caregivers.

Specialists: 

  • Coordinate between patient, home care providers and the medical team to ensure the right equipment is in the home. 
  • Evaluate home equipment needs to improve patient care and safety.

Regional Health Home+ Home Infusion

Regional Health Home+ Home Infusion specializes in providing medication and supplies for home use for children and adults. Infusion therapy is used to administer medication, nutrition and hydration. 

  • Home+ pharmacists, nurses or dietitians coordinate home infusion therapy services, working closely with the patient’s own health care providers.
  • Specialists teach patients and caregivers to properly and safely administer infusion therapy at home or Home+ Home Health providers may administer home infusion.
  • Medications and supplies can be delivered to a patient’s home free of charge, or supplies can be picked up at the Rapid City location. 
  • Home+ specialists work with insurers to determine coverage and any out of pocket expenses.

Hospice provides comfort care to terminally ill patients in their homes, nursing homes, hospitals, other supervised living facilities and in a hospice house. 

Compassionate, skilled providers work with  patients and families to meet physical, emotional, social and spiritual needs for the best quality of life near the end of life. Care and support are provided by an experienced team that could include nurses, social workers, nursing aides, physical therapists, counselors, chaplains, physicians, pharmacists and volunteers.

What is hospice?

Hospice is a philosophy of care that provides medical and supportive services to those who have a shortened life expectancy and who prefer to die without life-extending measures. Hospice care does not treat the underlying illness. Likewise, hospice care doesn’t hasten or prolong death. 

Hospice patients benefit from substantial relief from pain and other symptoms. Patients and their families are supported by a caring team, allowing the patient more control over the final stages of life. 

What care is provided? 

The Home+ Hospice team prepares an individualized care plan that may include pain relief, physical or occupational therapy and counseling support for you and your family. For patients at home, family and loved ones provide most of the hands-on care. Hospice staff visit regularly and are always available to answer questions. 

How does hospice manage pain? 

Our physicians, nurses and pharmacists are experts in managing pain so you are comfortable, yet alert, and able to enjoy each day to the fullest extent possible. 

Where can I receive hospice care?  

You can receive hospice care in your home, a nursing home, assisted living facility, the Home+ Hospice House or the Hospice Suite at Sturgis Regional Hospital.  

Who provides the care?  

Your team could include nurses, social workers, nursing aides, physical therapists, counselors, chaplains, physicians, pharmacists and volunteers. If you are at home, family or loved ones will provide much of your hands-on care. Patients who live alone or don’t have family available to assist them can receive care at the Hospice House, or at designated nursing homes. 

Who should consider hospice care?   

Hospice care can be provided when it is believed a patient has six months or less to live. Hospice serves terminally ill people of all ages, with all types of illnesses.  

What is the admission process? 

Talk with your physician about a referral for hospice care. Or, call Home+.  We will answer your questions and confirm your doctor’s orders. Home+ staff will help if you don’t have a physician. 

Once you sign consent and insurance forms, Hospice staff will immediately set in motion all the care you need to be comfortable.  

Do I need any special equipment at home before hospice care begins? 

No. Home+ care providers will assess your needs and help make arrangements for a bed or other medical equipment. 

Does hospice help the family after the patient dies? 

Home+ Hospice provides support for caregivers for at least a year following the death of a loved one. 

Is hospice care covered by insurance? 

Medicare Hospice Benefit covers the full scope of medical and support services for a life-limiting illness. Medicaid covers hospice in nearly all states. Most private insurance providers cover hospice care. Patients and families should verify their coverage. Home+ staff is available to help.  

How are hospice services provided and covered for nursing home patients? 

Home+ Hospice will oversee all of your medical care and provide services and equipment needed to manage your terminal illness if you are in a nursing home. Medicare and most insurance pay for hospice care in nursing homes. Patients will be responsible for the nursing home's daily room and board fee. For those on Medicaid, the room and board charge will be covered by Medicaid. 

Can a hospice patient who shows signs of recovery resume regular medical treatment? 

Yes. You can be discharged from hospice and resume medical treatment.

Hospice House

Hospice House offers a home-like setting for patients and their families. Hospice House provides three levels of care.

Acute care: The Hospice House provides 24/7 skilled nursing care for patients who are rapidly declining or where the medical condition requires skilled care.   

Respite care: A five-day stay is available under Medicare, Medicaid and some insurance to allow family members a respite from caregiving.  

Residential care: If hospice care at home isn’t possible, Hospice House provides residential care. Most insurance covers the hospice care at Hospice House. Patients are responsible for a $200 daily room and board cost.

Hospice House welcomes patients and families considering hospice care. Please call in advance for an appointment. 

Visits by family members and friends are welcome at the patient’s discretion. Friends can send mail, flowers and Mylar balloons.    

Since opening in 2007, Hospice House has offered home-like amenities, care and comfort for patients and their families. 

Comfortable rooms 

Private rooms are 24 feet by 15 feet and include a single bed for a guest.

Gorgeous views 

Each private room has a deck or a patio. Low windows offer beautiful views from the pillow.

Great room 

Families and friends can gather and relax in the great room.

Chapel and library

A chapel room and library are open for reflection, reading or work.

Shared kitchen

Family members can use the shared kitchen. Patients order meals off menus from Rapid City Regional Hospital.

Guest room

A guest room is available for families who wish to spend an evening at Hospice House.

Support and Counseling Services Offered 

Support and Counseling Services offers services for patients and family members. Staff members make contact with hospice families for at least a year following the death of their loved one.   A free support group is open to those who have lost a loved one. Be a Hospice Volunteer 

Volunteers play a special and important role in hospice care. A volunteer’s visit may be the single most important event in that patient or family member's day. The primary duty of the volunteer is to provide practical help with caring, warmth and understanding. Volunteers also assist with clerical and office support.

Specialty Pharmacy Enrollment 
Enrollment is required for Home+ to work on your behalf. Call Home+ Specialty Pharmacy to enroll, or ask your physician to phone, fax or e-prescribe on your behalf.  There is no charge.  

Enrollment benefits 
The Specialty Pharmacy team will:  

  • Work with insurers, including Medicare, so you know your coverage.
  • Obtain any needed prior authorizations from your insurer. 
  • Work to find resources for financial hardship, if needed.
  • Ship medication to your home at no extra cost.
  • Provide essential equipment such as alcohol swabs and sharps containers at no extra cost.

Therapy Management 
A Home+ nurse or pharmacist work with you one-on-one to make sure you receive the most benefit from your therapy. Patients receive:

  • Check-in calls from a therapy management nurse.  
  • Information about the safe and effective handling and storage of your medication.
  • Education on how to take or inject your medications correctly.
  • Clinical guidance to help prevent or manage medication side effects and injection site reactions.

Pharmacy services

New prescriptions: Contact us by phone or email. Or your doctor can submit your prescription to us.

Refills: Pharmacy staff calls you with refill reminders. Or contact us to request a refill.

Prescription transfers: The pharmacy staff will transfer prescriptions to or from Home+ Specialty Pharmacy.

Why use Home+ Specialty Pharmacy?

  • Home+ Specialty Pharmacy is the only pharmacy of its kind in the region, providing medication and therapy management for patients with chronic, rare or complex conditions that require specialty medications.
  • Home+ clinical specialists teach you about your medication, how to take it, how to minimize or manage side effects and how to store it. These one-on-one meetings may take an hour or longer.

Will there be a co-payment for my medication?
The co-pay amount is determined by your insurance plan.

Can I get financial assistance to pay for my specialty medication?                                 
The Specialty Pharmacy team will help identify patient assistance programs through drug manufacturers or other sources, if needed. 

Can I have all of my medications filled at Regional Specialty Pharmacy? 
Yes. We can fill all your prescriptions and help you with transfers from other pharmacies. 

How long does it take to receive my medication?
Prescriptions are available within one to two days once insurance approval is secured.

How do I get my medications? 
We will ship your medication, free of charge, within 24 to 48 hours. The pharmacy staff will contact you to arrange a convenient delivery time. A signature is required to deliver specialty medications.

How should I store my medication?
It’s important to follow the storage directions for your medication. Some specialty medications require refrigeration. Others should be stored away from direct sun or moisture. 

What if I experience a reaction to my medication?
Call 911 immediately with emergency concerns related to your medication. It is important that you follow your prescriber’s directions for taking your medication and discuss any concern you may have.  Contact your provider or the Home+ pharmacy team for any non-emergency questions, or if you experience any side effects that prevent you from taking your medication.

Can I return any unused medication?
Unused medications cannot be returned. Check the FDA WEBSITE for details about safe medication disposal or contact your city or county about take-back programs.
If you have any concerns about the condition of your medication when you receive it, please contact Home+ Specialty Pharmacy immediately.
